Medical PE Modification form due to injury
California Education Code section 51210(g) requires all elementary students to participate in physical education (P.E.) for a total of 200 minutes each 10 school days. Education Code section 51222 requires all secondary students to participate P.E. for a total of 400 minutes each 10 school days. California Education Code section 51241 states “The Board of a school district … may grant a temporary exemption to a pupil from courses in physical education, if the pupil is ill or injured and a modified program to meet the needs of the pupil cannot be provided.”

INDEPENDENT PHYSICAL EDUCATION (IPE) PROGRAM FOR MIDDLE AND HIGH SCHOOL ATHLETES
The deadline for IPE applications is August 8, 2025, at 12:00 p.m.
Capistrano Unified School District has created a new process for the Independent PE application submission to make it easier for parents or guardians to track the application process. The IPE class is run through the California Preparatory Academy, which is part of CUSD. This process makes it easier for the school and instructor to view completed applications and set up the online class.
Parents or guardians, please complete this form with your student. There is a workflow to this process that will be as follows:
1. Students initiate the form using their CUSD email address. Students will complete the following:
Student’s Name
Student CUSD ID Number
Telephone Numbers (students and parents)
Address
School they will be attending for IPE
Once the basic information is completed and the student signs the contract, they will move to the next step, which is to forward the form to their parent (this is done within the form).
2. Parents or guardians will receive an email from Infomed K-12, which will take them back to the form to complete the following sections:
Sport
Name of Coach/Instructor
Group/Club Affiliation
a. Evidence of Achievement: Please attach to this document a statement on official letterhead identifying the classification levels of the governing body and where the student ranks within those levels (see Exhibit A).
b. Statement of Reason(s) for Requesting Independent Study – Physical Education:
c. Please list a minimum of three mid-year goals that are aligned with the California Physical Education Standards
d. Please list a minimum of three year-end goals that are aligned with the California Physical Education Standards
Statement or list of daily activities by which the student will achieve the mid-year and year-end goals:
The parents or guardians will then sign the contract and consent to share this information with their coach or instructor. The parent will then forward this to their coach or instructor. A valid email address is needed. Please communicate with the coach or instructor that they will receive an email from Informed K-12.
3. The coach or instructor will receive an email from Informed K-12. The coach will verify that the information is correct and sign the contract. They will next select the school from a drop-down menu that the student attends. The school will be on the application, but it is recommended that the parent communicate with the coach or instructor which school the athlete will receive IPE.
4. The school Assistant Principal will receive the documentation and review it for approval.
